Please send the completed applcation along with any sales tax exemption certificates to our Finance/Credit Department by email to accountsreceivable@ottobock.com or fax to 512 806 2154. Technical Orthopedics Sales and Distribution Statement

Otto Bock HealthCare’s U.S. Technical Orthopedic business sells solely to Prosthetic and Orthotic facilities who employ certified and/or licensed prosthetists and orthotists who fit the device(s) to the patient/user.

In addition, there may exist product specific conditions where specialized training is required in order for a certified and/or licensed prosthetist or orthotist to become qualified to purchase, and consequently to custom fit, the product to the patient/user. 

Otto Bock sells its products only to those that are directly involved in clinical assessment and fitting of the product(s) to the patient/user, with the exception of Prosthetic and Orthotic schools.  There is no transfer of Otto Bock HealthCare’s limited warranty for any products that have been resold or redistributed.
